Answering and Submitting Self-Audit Questionnaires

Self-Audit Questionnaires help provide the Manufacturer with visibility into adherence to quality requirements across your operations. This guide will show you how to answer and submit the questionnaires requested from you.

1. Viewing Questionnaires
To see what self-audit questionnaires you have outstanding, click the first link under Self-Audit Questionnaires.
This will take you to a separate page with a list of all questionnaires you need to complete. For each outstanding questionnaire, the number of questions awaiting responses and the current completion status will be displayed.
By default, the list is sorted by customer name and is displayed in ascending alphabetical order.
2. Answering Questions
Before filling out a self-audit questionnaire, we recommend checking reference documents provided by the Manufacturer. They may contain useful guidance for completing the questionnaire.
To complete a questionnaire:
- Click the magnifying glass icon under Answer Outstanding Questions.
You will be taken to a new page. - Answer each Question listed.
Choosing some answers will generate a pop-up asking for additional information.
- Add Optional Comments if needed.
Once you click Comment, your comment will be stored, and you will not be able to modify or delete it. - Click Next & Save to proceed to the next set of questions.
You can go back to questions you've answered on the previous page by clicking Back & Save.
On the final page of questions, the Next & Save button will be displayed as Save & Close.
Once you have answered every question (or selected N/A for questions that do not apply to your site), the system will automatically take you to an "Approve and Submit" page, where you can review your answers and optional comments.
From here, you can also Download or Print the questionnaire for your records.
When you are ready to submit the completed questionnaire, click Click here to submit the questionnaire.
Not ready to answer every question listed?
You don't have to answer every question to save your progress.
For example, if you have only answered the first five questions on a page, you can click Update list of outstanding questions to store the answer and update the Number of Questions Outstanding count on the overview page.
3. Changing Answers
Whether you accidentally skipped a question or have additional information to provide, changing answers is easy!
To change an answer:
- Click the pen and paper icon under Change Answers.
You will be taken to a new page with a folder tree containing your answers. - Select + next to the questionnaire name to see its sections.
- Choose the section containing the answer you need to modify.
Continue this process until you see questions listed.
- Select the question with the answer you want to change.
You will be taken back to the page, with your answers listed. To get back to the folder tree, click Back to all questions. - Make the required changes.
- Click Save & Close.
In this example, we changed the job title for the person who completed the questionnaire.
4. Exporting Questionnaires
To download a questionnaire, click the download icon under Export as XLS in the questionnaire overview.
